Here are the patches:

" I did some checking here and noticed that a couple of traces were dropped.

You'll need to add a jumper on the back of the PCB between pin 1 of K2 and
pin 3 if U1. 

Also a jumper between pin 4 of U1 and R14 pull up resistor on
the pin not connected to VCC. 

I am not sure why those traces were missed
but it is an easy fix. 

Installed a jumper at K2 between pins 1 and 2 to
connect the output of the PTM (O1) to the /NMI line via the U1 inverter.   "

Also a jumper is needed between pin 1 and 9 on the 6551 to correct CTS issue between PC serial and I/O mezzanine.

Need to add jumper for reset if backplane is being used.
